The Conference Registration Packet with all information can be downloaded as a Word Document (.doc) or as a PDF Form (.pdf) .

The Conference Registration Packet includes:

1. Conference and Dinner Registration Form (Required)
2. REFORMA Membership Application (Optional)
3. UA Coronado Hall Individual Reservation Form (Optional)
4. REFORMA Outreach Fair Participation Form (Optional)

You have two ways of registering!

1) ONLINE: Fill out the Conference and Dinner Registration Form online. However, the optional forms (2, 3 & 4) in the Conference Registration Packet must be mailed in.
