How to join REFORMA and the REFORMA Tucson Chapter:

1. Fill out the REFORMA Membership Form; Please note! You only need to fill out one application form to join both REFORMA National and REFORMA Tucson — just remember to check the box for the “Tucson” chapter.
2. Mail your form and payment to:
